As the population ages and the number of older adults continues to grow, chronic illness becomes more prevalent and greatly impacts our health care system.  Read the article about Self-Management and Chronic Disease. As a nurse, how can you help patients achieve and maintain a high functioning level when living with a chronic and/or debilitating illness.
