Reflection Question: At this point, what grade would you give yourself and why?

This patient looked pale and pretty weak. The nurse came over to complete the patient’s assessment and confirmed that she was now considered an active patient. The nurse, social workers, and chaplain thoroughly educated the patient daughter and her sitter on what to expect while the patient was transitioning, and we made sure they had the on-call number to call if they had any questions or concerns. We went to see another patient who was a new admit. This patient was 76 years old male with cancer that had spread all over his body. The patient is living with their daughter in her apartment, and he states he wants to be able to get out of bed again. We had to educate the patient and his daughter on how things go with hospice because the daughter thought that we should get physical therapy set up for the patient and a couple of things regarding his medications she was concerned about. She thought everything would be the same process, but now she understands how hospice works. This patient now understands if he attempts to get out of bed by himself, he could fracture a bone with him being very fragile, his daughter states he lost about 50 lbs, and he wasn’t eating much, but he pretty much sleeps all day. We had several patients we went to see in two of the local nursing homes so we were pretty busy this week.
We had two placements we had to check the status on for the nursing home, one was pending additional progress notes so we called the office so they could fax it over to the nursing home and for the other one we are waiting for the nursing home to follow up but they have everything they need.
