This week we did several home visits and assessments. We had one patient who told his caregiver, his nephew, that he wanted to go to a nursing home. The patient’s nephew told one of the nurses that when the nurse came to visit, the nurse noticed that the hospital bed that Kindred Hospice provided was gone. The nurse reported everything to my field instructor, and we called and went by the patient home, but no one was available. We will work on educating the patient and caregiver on the process for nursing home placement. We are also working on finding out where the bed is so we can return it to Kindred Hospice. The patient or the nephew have not returned any calls, so we will continue to follow up. We had another patient we would place in the nursing home a couple of weeks ago, but her husband decided he did not want a lien on their home. Well, the husband called, stating his wife, the patient, wandered out of the house, walked to the highway, sat in a chair trying to hitchhike for a ride, and attempted to get in a man’s truck. This patient has dementia, so she had no clue what she was doing. Her husband is now wanting the patient placed in a nursing home. We notified the nursing home to see if the referral that was placed a couple of weeks ago was still good, but we were told that we would have to send another referral in, so we worked on that, and we are now waiting for the patient’s husband to complete a Medicaid application and paperwork for placement for the nursing home. We called to verify placement for respite care on another patient who went to stay at the hospital for five days. Everything was good to go pending time for transportation since the ambulance was transporting the patient. This has been a pretty steady and productive week. I feel like I am now able to react to situations as they occur, and it all feels pretty normal to me, if that make sense. My instructor seems to think I am doing an outstanding job!
