Remember the role-play exercise from Week 2? Great! This week you will review your own session and complete a SOAP note like the one you reviewed in this week’s materials.

Instructions:

1) Review your client session from Week 2

DON’T WORRY IF YOU DON’T HAVE IT SAVED, YOU CAN GO BACK INTO YOUR SUBMISSION TO VIEW IT AGAIN

2) Download and complete the SOAP Note Template & Instructions-1.doc Download SOAP Note Template & Instructions-1.doc

3) Upload your completed document. Be sure that I am able read your note. If at all possible, type your SOAP note, rather than hand write it. In the field, you will typically be using electronic records. Good documentation is not useful if it cannot be read.

Requirements: 1-2 PAGES

I will have to do SOAP Notes on a video that I cant get posted to you for some reason not sure but I will review the video sometime and write the notes and send to you .and you can tidy it up for me and I will send another form.
